Welcome to the Fantrax MLB Injury Report for Friday, April 25, 2025. In today’s edition, Cole Ragans gets lifted early for groin tightness. Also, Tyler O’Neill might need additional time off for his neck, while Ketel Marte could be back sooner than expected. All of this and so much more in today’s Injury Report from Fantrax.
MLB Injury Report
Cole Ragans – SP Kansas City Royals
Cole Ragans exited early after 3.0 innings in today's first game of a double-header with groin tightness.

Ragans lasted just three innings today and threw just over 60 pitches in his start against the road Rockies. It was disappointing on many levels, one of which was the poor numbers posted, hurting fantasy managers everywhere. Additionally, his injury leaves these disappointed managers in the dark because he will undergo testing to determine the severity of the issue. Chances are that he will need some time on the Injured List, or at the very least, miss his next start. However, if it is anything more than a mild strain, these fifteen days could easily double.
Arizona Diamondbacks – Team Update
Still nothing on A.J. Puk.
Kevin Ginkel today in Reno threw 22 pitches.
Ketel Marte keeps making strides. Torey Lovullo said he does not think Marte is 2 weeks away, hopefully sooner. pic.twitter.com/0aaA4RTM2Y

The lack of news or updates on AJ Puk is concerning, as he was looking at multiple opinions on his ailing elbow. It feels like the longer this issue lingers, the worse the news will be, but the hope is that I am wrong. As for Ketel Marte, manager Torey Lovullo laid out a nice, solid game plan for his return, stressing that he and the team do not want to fall into the trap of believing that he is ready when he is not. They did that last year with a few of their players, and it feels like they have learned their lesson. It remains to be seen if he can return in under two weeks, but it’s nice to know that the team sees some progress.
Tyler O’Neill – OF Baltimore Orioles
Brandon Hyde said Tyler O’Neill isn’t in tonight’s lineup with the same neck soreness he dealt with last week.
Hyde is hoping O’Neill can be available off the bench tonight.

O’Neill has been in and out of the lineup with this recurring neck issue. If he continues to do this, then it might be time to place him on the Injured List before the ailment becomes too much to bear.
Royce Lewis – 3B Minnesota Twins
3B Royce Lewis will begin a rehab assignment with St Paul on Friday, Twins announce.
— Phil Miller (@MillerStrib) April 24, 2025
Chances are that this rehab assignment will last at least a week, as Lewis did not have much of a spring training to build and grow. As such, he will need to work on getting his timing down, as well as the mechanical issues of being at the plate. Regardless of all of this, the good news is that he is ready to get into game-type situations and may be able to join the Twins by the beginning of May.
Willi Castro – INF Minnesota Twins
The Twins placed Willi Castro on the 10-day IL with an oblique strain. It’s retroactive to only April 21 because the Twins tried to avoid putting him on there in Atlanta.
Mickey Gasper up and David Festa was optioned to Class AAA with Pablo López expected to return Friday.
— Bobby Nightengale (@nightengalejr) April 24, 2025
The Twins have sent another part of their lineup to the Injured List, this time versatile utility man Willi Castro. They are running dangerously thin on options here and are forced to call up people who may not be ready. This was a problem a few years ago and is something that they’re experiencing now.
Kumar Rocker – SP Texas Rangers
Kumar Rocker is going on the injured list with mild right shoulder impingement, per Bruce Bochy
— kennedi landry (@kennlandry) April 24, 2025
This could help explain his poor outing earlier in the week, as Rocker was rocked (pun intended) for five earned runs in just under two innings of work. As someone who throws hard, it’s not surprising that he is having some arm difficulties here. The team has yet to give an update on when they plan to have him back in the rotation, but chances are this trip to the Injured List lasts longer than the fifteen-day minimum.
Paul DeJong – SS Washington Nationals
Paul DeJong saw an eye specialist today who found no issues there. DeJong will now undergo surgery to repair his broken nose. Davey Martinez didn’t offer a timetable for his eventual return.
— Mark Zuckerman (@MarkZuckerman) April 24, 2025
There is no official timeline for a return, but one would have to think that his absence will be weeks, not days.
Zack Gelof – 2B The Athletics
Zack Gelof is progressing well and expected to begin his rehab assignment next week with Triple-A Las Vegas. Kotsay noted Gelof had nearly a full Spring Training of at-bats, so he may not require too long of a stint down there.
— MartÃn Gallegos (@MartinJGallegos) April 25, 2025
This is great news for the team, as they could use his bat and glove at second base. The fact that he may not need this to be a long rehab assignment means he could be back by this time next week, making him someone to bid on this weekend in FAAB.
Cade Cavalli – SP Washington Nationals
Cade Cavalli rehab start No. 2 in Class AA Harrisburg: 5.0 IP, 2 H, 0 R, 1 BB, 4 K. He was up to 69 pitches, 43 of which he threw for strikes.
— Spencer Nusbaum (@spencernusbaum_) April 25, 2025
Cavalli is returning from having had Tommy John Surgery over two years ago, as the Nationals took the slow road back with his build-up. Now, he looked to be almost ready to return, up to 69 pitches. It’s anyone’s guess when they will pull the trigger and let him pitch in Washington, but he is someone to keep stashed for the upside that he offers.
Tony Gonsolin – SP Los Angeles Dodgers
Dave Roberts said the goal was to get Tony Gonsolin thru five innings tonight. He is likely to rejoin the Dodgers next week https://t.co/PFUNhp24Sa
— Jack Harris (@ByJackHarris) April 24, 2025
And boy, do the Dodgers need him. Gonsolin arrives at the right time here for a team that desperately needs healthy arms.
