Welcome to the Fantrax MLB Injury Report for Monday, May 5, 2025. In today’s edition, Triston Casas suffered a gruesome season-ending injury that required surgery over the weekend. Also, Randy Aronzarena and Shota Imanaga might miss some time with a hamstring strain, but is it too early to tell? Finally, we have some positive news, as George Kirby took his first step toward a return while Royce Lewis completed his last. All of this and so much more in today’s Injury Report from Fantrax.
MLB Injury Report
Triston Casas – 1B – Boston Red Sox
Now official from the club:
Red Sox 1B Triston Casas today underwent a successful left patellar tendon repair. The procedure was performed by Dr. Eric Berkson at Massachusetts General Hospital.

The biggest (and most heartbreaking) news from the weekend was seeing that Triston Casas would miss the rest of the season due to a ruptured patellar tendon in his left knee. The good news is that his surgery was a complete success, and he can begin working on returning healthy for next season, though there has been no timeline offered just yet. In his absence, Romy Gonzalez has started both games at first base, but it feels like that won’t be the long-term answer. While Rafael Devers could slide over into this role, there’s also the chance that Boston trades for their future first baseman, as they will be contending for a World Series.
Shota Imanaga – SP Chicago Cubs
#Cubs aren’t sure yet about the severity of Shota Imanaga’s left hamstring injury. He will get imaging tomorrow. They’ll wait for results to determine next steps.
— Taylor McGregor (@Taylor_McGregor) May 4, 2025
It was an unfortunate exit, as Imanaga had what looked like a quality start until the injury. Looking ahead, it’s fair to compare this injury to that of Spencer Strider, who was placed on the Injured List and has yet to resume throwing after two weeks. If Imanaga has the same fate, he could be a candidate to return in June. That being said, we will have to wait for the results to figure out any other type of plan.
Randy Arozarena – OF Seattle Mariners
Randy Arozarena left the game with “a little bit of a hamstring thing … but nothing serious,” Dan Wilson said.
— Adam Jude (@A_Jude) May 4, 2025
The good news here is that it would appear that the issue is a minor inconvenience, meaning he has a shot to be back in the lineup when the Mariners play today at Sacramento. Of course, with the team travelling overnight, it wouldn’t be surprising to see him sit out today’s game and return at some point during this series. That’s a long-winded way of saying he’s day-to-day.
George Kirby – SP Seattle Mariners
George Kirby looked great in his first rehab start. Final line: 3IP, 1H, 1R, 0BB, 4K. 42 pitches, 33 strikes. pic.twitter.com/y1OZOkexYG
— Mariners Minors (@MiLBMariners) May 4, 2025
This was a very encouraging first step into Kirby’s extended rehab assignment. His velocity topped out at 97.8 miles per hour in this start, and the 42 pitches thrown looked crisp and efficient. How he feels after this start as he prepares for his next one will be key, as his body adjusts to throwing again, hopefully without pain. He will likely need at least three more starts before making his 2025 debut with the Mariners.
Yordan Alvarez – OF Houston Astros
Yordan Alvarez is feeling better, but the Astros are going to stay away from him today. Joe Espada said the team doesn’t think imaging is necessary for his right hand.
— Chandler Rome (@Chandler_Rome) May 4, 2025
Alvarez missed games on both Saturday and Sunday with right-hand inflammation. The hope is that this is a minor setback and that his time away will be minimal. It’s worth noting that he suffered from this same injury in the summer of 2022 and missed eleven days as a result.
Anthony Volpe – SS New York Yankees
Anthony Volpe #Yankees is not in the lineup today but the x-rays and MRI were encouraging. He's listed as day to day.
— Jim Bowden⚾️ (@JimBowdenGM) May 4, 2025
The scary part of this situation is that Volpe said that he heard a “pop sound” in his shoulder while playing on Saturday. The clean X-rays are an encouraging sign that both he and the Yankees have dodged a bullet here, but until he’s back in the lineup, fantasy owners can’t necessarily breathe any sigh of relief.
Masyn Winn – SS St. Louis Cardinals
#STLCards SS Masyn Winn left the game in the fifth inning with a left ankle sprain. Winn twisted his ankle in the third inning when he tripped over third base while deftly avoiding a collision with #Mets third baseman Mark Vientos.

The updates after the game were sparse, possibly because the Cardinals have a game today to get ready for, but we should know more about this injury before lineups lock tonight. Fantasy owners would be wise to find an alternative at shortstop just in case a trip to the Injured List is warranted.
Royce Lewis – 3B Minnesota Twins
#MNTwins are off Monday after going 8-5 over a stretch of 13 games in a row. Before all the Triple-A rainouts, hope was Royce Lewis would be ready to join them on Tuesday.

Be prepared!
Tanner Bibee – SP Cleveland Guardians
Guardians starting pitcher Tanner Bibee is exiting the game here in the sixth. He came out to warm up, but signaled to trainers and is being lifted in favor of LHP Tim Herrin. Appears to be a cramping issue, something that Bibee has dealt with in the past. Will update. pic.twitter.com/5SZ5phyUCW
— Joe Noga (@JoeNogaCLE) May 4, 2025
Bibee was having a successful outing on Sunday when he was suddenly pulled in favor of these cramps. The good news is that he has dealt with issues in the past, which haven’t affected his ability to be a solid pitcher. The hope is that with proper hydration, he will be able to overcome this hurdle and remain on the mound for as long as he’s needed in his next outing.
Otto Lopez – 2B Miami Marlins
Marlins say 2B Otto Lopez has a right ankle sprain. Grade 2. Hurt it yesterday pre-game. Sounds like IL.
— Craig Mish (@CraigMish) May 3, 2025
Lopez was eventually placed on the Injured List, and the belief is that he will miss approximately two or three weeks of action. In the meantime, the Marlins are quite thin for quality depth, as Javier Sanoja got the nod for both games thus far at second base. It remains to be seen if Xavier Edwards will get a shot there as well to give others a chance at shortstop.
